Transcript Request
You may request a transcript via fax, mail or in person.
Please complete the Transcript Request Form (PDF), or write a letter including the below information:
- Full Name, including maiden
- Social Security Number
- Phone Number
- Date of Birth
- Complete address where transcript is to be mailed (or a pick-up date)
- Number of copies (limit 5 per request, please)
- SIGNATURE and date
*Transcripts are processed within 24 hours of the request (except holidays and weekends).
