Guidelines for a Letter Request
You may request an official letter via fax, mail, or in person.
Please complete our Letter Request Form (PDF), or write a letter including the following information:
- Full Name
- Social Security Number
- Phone Number
- Date of Birth
- Detailed information about what you need the letter to verify
- Complete address/fax number where letter is to be sent (or pick-up date)
- Number of Copies
- SIGNATURE and date
*Letters are processed within 48 hours of the request (except holidays and weekends).
