Guidelines for Changing Personal Information
All updates/changes to personal information, including name, address, phone number, etc. must be done in writing; verbal changes will not be accepted. **All name changes must be submitted with proper documentation (i.e. marriage certificate, divorce decree, or court document). Please note that a driver’s license is not a valid form of identification for a name change.
You may use our Change of Name/Address Form (PDF) , or write a letter including the following information:
- Full Name
- New Name (if applicable)
- Social Security Number
- Date of Birth
- Previous Address
- New Address (if applicable)
- Phone Number
- SIGNATURE and date
